This issue exemplifies that approach.

Since publication of our last issue of Insulin, the American Diabetes Association Scientific Sessions were held in San Francisco, California. The results of 3 landmark studies (Action in Diabetes and Vascular Disease [ADVANCE], Action to Control Cardiovascular Risk in Diabetes [ACCORD], and Veterans Affairs Diabetes Trial [VADT]) were revealed, all of which focused on the role of glucose control and its influence on macrovascular disease in type 2 diabetes mellitus (DM).

This issue of Insulin develops the theme of insulin therapy for both hospitalized and ambulatory patients. Recent evidence strongly suggests that intensive insulin therapy for hospitalized patients with hyperglycemia, whether or not they have diabetes mellitus (DM), significantly reduces morbidity and mortality. The excitement following these studies has led to major changes in DM management in hospitals worldwide. In most intensive care units, protocols have been developed and instituted to follow the guidelines suggested by the latest studies. We all eagerly await outcomes data from our own institutions, but early indications are positive.

Self-monitoring of blood glucose (SMBG) and insulin therapy are 2 crucial components that must be used together to achieve glycemic goals. SMBG is an important self-care tool that can motivate patients to take a more active role in managing their diabetes. It can provide the information needed to make rational and safe clinical decisions regarding insulin adjustments, as well as the patient's overall diabetes care. This issue of Insulin focuses on SMBG and insulin therapy, with continued emphasis on both the fasting and postprandial values.

Each issue of the journal Insulin covers important aspects related to clinical diabetes mellitus (DM) and its management. The current issue addresses a very important condition called latent autoimmune diabetes in adults (LADA) that is often missed in the clinical setting. The focus here is the presentation of type 1 DM in adults who may have been previously diagnosed and treated as if they had type 2 DM. The article describes the underlying pathophysiology, the clinical presentation, and the treatment of LADA.

The transitioning, in a timely and efficient manner, of patients who are not achieving glycemic goals using oral antidiabetic agents alone to treatment with insulin is unquestionably one of the greatest challenges in diabetes care. We are excited to present this issue of Insulin that focuses not only on practical insulin treatment strategies but also highlights many of the barriers that prevent adequate diabetes care in this country.

In this first issue, we have recruited articles by experts on topics such as hypertension in the patient with type 2 DM, guidelines for glycemic control, the benefits of early initiation of insulin therapy, and how to overcome the psychological barriers to insulin use. Another important aspect of the journal is that each issue will contain special features such as the Patient Handout (the first one discusses injecting insulin) and an interactive Case Study that reinforces the information presented in the articles.
